A preventative care membership for those who value proactive health, early detection, and whole-person support. Focuses on prevention, education, and structured health coaching over prescriptions.
Includes:
- 1 Comprehensive physician visit/year (virtual or in-person, 45-60 mins)
- 48-hour scheduling for urgent needs
- 1 Wellness Coaching session/month
- 1 Mind–Body Alignment session/month
- 1 Nervous System & Energetic Regulation Session/month
- 15% off all Birdsong services and treatments
Membership Terms & Policies
Birdsong operates as a low-volume, high-touch concierge practice with limited membership spots. All tiers require a 6-month minimum commitment. Members may cancel after the minimum term with written notice.
Cancellations
- 30 days’ written notice is required after the commitment period ends.
- Notice must be submitted via email or through the member portal.
- Membership remains active and billable during the notice period.
- Early Cancellation Fee: $500 per person for cancellations before the 6-month minimum term is fulfilled.
- All membership fees (monthly, annual, or prepaid) are non-refundable.
Membership Freeze Option
- One freeze per year
- Up to 60 days
- Permitted for medical or travel reasons
- Freeze fee: $30/month
Automatic Renewal
All memberships renew month-to-month after the initial commitment period unless cancelled in writing with 30 days’ notice.
Billing Authorization
Member authorizes recurring billing for dues. Failed payments may result in temporary suspension until resolved.
